Claim damages form for corporate entities

To ensure smooth processing of your compensation claim, we ask you to review the instructions below, carefully complete the compensation claim form, and attach the required documents to verify the value and damage of the goods. Read more detailed instructions here.

Follow these steps:

  1. Complete the compensation claim form carefully.
  2. Include proof of the value of the damaged or lost goods in the form of a purchase invoice. Attach a repair estimate if the damaged item is repairable.
  3. Attach a copy of the credit invoice if compensation is made to the recipient and the goods remain with your customer.
  4. Weigh the damaged item without packaging materials or specify the weight of the lost item or its part in the compensation claim.
  5. Photograph the damaged goods and all packaging materials:
  • photograph the outer packaging completely, including any possible damage to it
  • photograph the damage to the item and the damaged product as a whole
  • take a picture of the opened contents, ensuring the item and the protective materials inside the shipment are visible
  • photograph the packaging from the inside without its contents so the interior of the box is fully visible
You can attach files to your claim for damages up to a maximum of 5 MB. If the attachments exceed the maximum size, please indicate this in your claim. You will receive instructions on how to submit the attachments during the processing of your claim. Submitting the claim is not possible if the information is incomplete or if the attachment size limit is exceeded.
